Undoctored Suggested Labs

Edition: 2017-10-01

The following table summarizes various medical lab tests that are suggested in Undoctored and Wheat Belly publications, along with target results ranges (where found), and alternate units of measure.

The target values here may be expect to vary from both the Reference Ranges provided by labs, and any assessment of “low”, “normal” or “high” provided by a lab or consensus physician.

For UIC and Cureality members, many of these may be tracked and rated on the Cureality Tracking Dashboard. Enter data, 6 values per page load, on the Health Tests Manager page.

Some of these numbers may be subject to only very indirect control by diet and lifestyle. Advice, or links to advice on control are not so far part of this summary.

B   part of a Basic workup (e.g. annual physical)
  C   part of a Cardiac workup
  M   part of a Metabolic workup (metabolic syndrome through diabetes)
  T part of a Thyroid workup
B C M T Lab Target
  Blood Glucose
B C M - HbA1c 4.0 to 5.0% (NGSP)
20 to 31 mmol/mol (IFCC)
68 to 97 mg/dL (eAG)
5.3% or higher requires attention.
B C M - Fasting (FBG) 70 to 90 mg/dL (3.9 to 5 mmol/L)
B C M - Postprandial (PPBG) 100 mg/dL (5.5 mmol/L) or less, but ideally no rise from FBG
- - M - Fasting Insulin Ideally 0
Attention indicated above 3 µIU/mL (21 pmol/l)
  Blood Pressure
B C M - Systolic 90 to 115 mmHG (12 to 15.3 kPa)
(115/80 mmHg and lower, but not much lower than 90/60 mmHg, or 15.3/10.7 kPa and lower, but not much lower than 12/8 kPa)
B C M - Diastolic 50 to 70 mmHg (6.7 to 9.3 kPa)
  Inflammation and Calcification
- C - - Calcium Score (Agatston) Zero. If non-zero:
Growth -100 to +10% per year (lower is better)
The CAC test is a “CT calcium” and not a CT angiogram. In some locales, EBT scans may still be available. Get at least a 64-slice in any case.
B C - - C-Reactive Protein (CRP or ls-CRP) 0 to 1.0 mg/L (and zero is fine)
Getting a precise reading below 1.0 might require hsCRP
- C - - Homocysteine Dashboard has: 0 to 10 µmol/L (0 to 1.35 mg/L)
  Lipids (Standard Panel)
Important: take this test fasting (you may be falsely told that’s not necessary).
B C M - Triglycerides (TG) Under 60 mg/dL
Dashboard has: 30-60 mg/dL (0.34 to 0.68 mmol/l)
B C M - High Density Lipoprotein (HDL) 60 mg/dL or higher
Dashboard has: 60-240 mg/dL (1.55 to 6.2 mmol/l)
B C M - Low Density Lipoprotein (LDL, actually LDL-C) “worthless”
DLDL, aka LDL-D is not much more useful. If someone insists on a simplistic number to gauge against consensus guidelines, use:
  LDL-P# ÷ 10
Dashboard has: 50 to 70 mg/dL (1.3 to 1.8 mmol/l)
B C M - Total Cholesterol (TC) “ignore it”
  Lipids (Advanced Lipoprotein Panel)
- C - - Apoprotein B 0 to 70 mg/dL (0 to 0.70 g/L)
- C - - B100/A1 Ratio 0.5 or less
Cureality Program Guide
- C - - IDL (intermediate-density lipoproteins) Dashboard has: Zero
- C - - LDL Particle Number 600 to 800 nmol/L
- C - - Small LDL Particles “no higher than 20% of total”
- C - - Large HDL Particle Number (HDL2b) Dashboard has: 4.8 to 24 µmol/L
- C - - Lipoprotein(a) <10 mg/dl (electrophoresis),
<30 mg/dl (VAP), or
<50 nmol/L (NMR)

Note: VAP is no longer available, but you might have older readings.
  Micronutrient Serum Levels
B C M - Magnesium (RBC Mg) upper half of lab “Reference Range,” or even slightly above it
(and not Serum Magnesium)
- C M - Omega-3 Index Dashboard has: 10.0 to 12.0%
- C M - Omega-6/Omega-3 Ratio 0 to 2:1 (Undoctored, page 191 of print edition)
- C M - Vitamin D (25-OH Vitamin D3) 60 to 70 ng/mL (150-180 mmol/L)
(and not 1,25-di-OH-vitamin D)
  Phenotype
- C - - Apo E (Apolipoprotein E) One-time genetic test: cannot be changed, but is important to know. If your out-of-pocket for the test is US$100 or more, consider getting a complete genome sequencing from 23andMe (Ancestry test suffices), which provides raw data that you can use to determine Apo E status (but 23andMe does not fully report it themselves, even on the Health and Ancestry test).
  Thyroid
Note: if on thyroid hormones, take daily dose after test.
- C - T Free T3 (fT3) Upper half of lab “reference range” (Undoctored, page 284 of print edition)
- C - T Free T4 (fT4) Upper half of lab “reference range” (Undoctored, page 284 of print edition)
- C - T Reverse T3 (rT3) Lower half of Reference Range.
- C - T Thyroid Antibodies:
TPOAb
TgAb
TSI
(TBII)/TRAb
Preferred tests are TgAb and TPOab. Target is within "normal" lab range, for both. A single antibody test is not dispositive on thyroid autoimmunity.
B C M T Thyroid Stimulating Hormone (TSH) 0.2 to 1.5 µIU/mL (Undoctored, page 284 of print edition)
  Metabolic
B - M - BMI (body mass index) Dashboard has: 18.5 to 24.5 kg/m2
(This is an extremely vague measure, but one you are likely to get routinely. It mostly falls apart for the very tall, and entirely falls apart for the very young and the extremely fit.)
B - M T Temperature (arising oral) 97.3°F (36.3°C) or higher, but not much higher (Undoctored, page 286 of print edition)
This would be done at home by you. If not asked for or asked about by your doctor, that may be revealing about the quality of care afoot.

Berkeley also has an Lp(a) with ref range of <30 mg/dl, although the test is different than VAP.

And once we have these numbers, how often does the testing need to be repeated?  Is it the same for all test?  Thanks.
