Posted: 8/13/2017 12:16:13 PM Edited: 8/13/2017 1:26:30 PM (3)   I’ve read all sorts of comments for years that if a person has a low fasting blood glucose level and a low HbA1c that they don’t have pre-diabetes or full-blown Type ll.  I learned first hand this week that it is not an absolute. Since 2010 I’ve tracked my FBG and A1c with a finger stick.  My FBG runs from 80-89 and A1C runs 4.8 - 5.2.  I also work very hard at following a VLC diet.  I am not a saint and will wander off once a month or so and slam a birthday dessert. My doctor, Dr. Ford Brewer of Nashville, TN doesn’t follow that train of thinking.  He runs PrevMed Heart Center in Nashville http://www.prevmedheartrisk.com and has a solid background in heart attack, stroke, diabetes and cancer prevention.  It’s a concierge practice and follows the Bale-Doneen Method of prevention. Dr. Brewer runs the FBG and A1c along with a 2-hour oral glucose tolerance test (OGTT) in addition to an extensive list of blood, urine, oral pathogen and genetic labs.  While my FBG and A1c came in very low, my OGTT test indicated a very high 1 and 2-hour postprandial glucose level after fasting and downing a solution of 75 grams of glucose.  My 2-hour postprandial level was even higher than the first hour.  I never thought all of this was possible.   He calls it age-related insulin resistance and finds that 50% of patients over age 50 have it and if left untreated the majority will typically go onto develop full-blown Type ll.  The resulting inflammation from circulating glucose can damage the endothelial wall.  He uses the microalbumin/creatine ratio as one indicators of endothelial damage and my ratio is extremely high. Here is what that looks like.
